1. Oversight & Supervision
Provide clinical oversight of care staff, including Direct Care Workers (DCWs) and CNAs.
Conduct periodic supervisory visits to ensure care is delivered according to the client’s plan.
Monitor staff performance, provide coaching, and document compliance with care standards.
2. Care Planning
Develop individualized care plans for clients based on assessments.
Review and update care plans regularly or when the client’s condition changes.
Ensure care plans are clear, practical, and communicated to caregivers.
3. Training & Education
Train new caregivers in safe practices, infection control, and emergency procedures.
Provide ongoing in-service training (e.g., dementia care, fall prevention, nutrition).
Verify that all staff maintain CPR/First Aid certification.
4. Client Safety & Quality Assurance
Conduct home visits to assess client safety and environment.
Ensure care practices align with state regulations and agency policy.
Investigate and document incidents, complaints, or changes in condition.
5. Communication & Coordination
Communicate with physicians, case managers, and family members as needed.
Report client status changes that may require medical attention.
Coordinate with other health professionals for referrals or additional services.
6. Regulatory Compliance
Maintain documentation that demonstrates compliance with state standards.
Assist with audits, surveys, and quality improvement initiatives.
Ensure adherence to HIPAA and patient confidentiality requirements.
